Martin Description: This patient was photographed in Guatemala, Central America, exhibiting a condition known as typhus gangrene of the right foot, which was determined to be due to a case of typhus fever, also referred to as epidemic louse-borne typhus caused by the bacterium, Rickettsia prowazekii. The dissemination of Y. pestis bacteria, predisposes plague patients to abnormal coagulation within the blood vessels of the distal extremities, including the eventual sloughing of the dead skin. The feet of this child reveal pitting edema, which occurs as a result of pressure applied by an examiner’s fingertips upon the skin, causing an indentation in the swollen, fluid filled skin that persists for a long period of time after the release of the pressure; note the pressure points in both feet of this child.
