Compare to #4 2. Trachea deviated to the right suggestive of tension pneumothorax. Note that even with this severe tension pneumothorax there is minimal tracheal deviation up in the neck. This helps to demonstrate the part of the reason for the poor sensitivity of physical exam findings for the detection of tension pneumothorax. 3. Depression and flattening of the left heimdiaphragm due to increased pressure within the left hemithorax. Also note the increased space between ribs. 4. Normal lung markings. Note the hyperinflation of the lungs (more than 10 rib spaces visible) and the flattening of the usual dome-shaped configuration of the diaphragms, also visible on the lateral view. This is consistent with "air trapping" conditions such as asthma and emphysema.
